In a staple bound chequebook style booklet with a blue card cover, it is dated 5th December 1945. Handwritten in black marker on the front cover is the number ' 6'. The counterfoils are numbered 51 - 60. The nominees are Richard Hosken, Arthur Jenner and Edward Tregenza.
